Over the past two weeks, a few requests have come into the Tobacco Dependence Resource Center for sample Patient Surveys. Surveys can be used to raise awareness about tobacco use in your patient community and also help the community to take ownership of the tobacco-free policies your programs develop. This is a rudimentary sample patient survey in Word format. Please feel free to use this sample, adapt it to meet your needs, and if you would be like to contribute to our Community of Learning and Action, send us an email with your version. We will post it to the website and share it on our mailing lists.
